Support Coordinator Role

This is a part-time SUPPORT COORDINATOR role that plays a key part in our team at Lets Connect Support Services.

We are seeking an experienced and compassionate individual to work with NDIS participants, their families, and carers to assess needs and coordinate supports.

The successful candidate will be based in Redcliffe QLD and will be responsible for providing high-quality support coordination services, including face-to-face visits and meetings with clients.

We pride ourselves on offering unique client-focused support to people with disabilities, their families, and carers.

In this role, you will have the opportunity to make a real difference in the lives of people with a disability and contribute to our company's mission to improve their lives and achieve their goals.

  • Assess the needs of NDIS participants and develop individualised plans to meet their goals
  • Coordinate supports and services to enable participants to live independently and participate fully in their community
  • Provide regular updates and reports to participants, their families, and carers
  • Participate in expos, team meetings, and networking meetings to promote our services and build relationships with stakeholders
Key Requirements
  • Certificate I - IV (preferred)
  • 3 years experience in the NDIS industry (preferred)
  • NDIS Worker Screening Card (required)
  • Driver Licence (required)
  • Australia Work Authorisation (required)
