Care Manager

We are seeking a passionate and experienced professional to join our team in South Australia. As a Care Manager, you will be responsible for managing relationships and overseeing service compliance for clients who have chosen us to fully manage their Home Care Packages.

The ideal candidate will possess excellent communication skills, be able to work effectively with clients and staff, and have a strong understanding of home care services. They will also be proactive in identifying and mitigating client challenges and ensuring a smooth home care experience.

Key Responsibilities:
  • Complete comprehensive assessments and care plans for clients
  • Mitigate client challenges and facilitate a seamless home care experience
  • Supervise care delivery to ensure high-quality outcomes
  • Review client progress notes and implement corrective actions as needed
  • Meet with prospective clients to explain the value proposition of our services
  • Report and manage incidents and hazards in accordance with company policies
  • Develop and maintain positive relationships with clients and their families
  • Collaborate with the scheduling team to ensure staff and client expectations are met
  • Chair regular team meetings to support staff in their day-to-day duties
  • Conduct performance reviews with your team to ensure everyone is working towards common goals
