Home Care Advisor

This role is a key position in our team, responsible for providing exceptional care to clients in their homes. As a Home Care Advisor, you will work closely with our clinical and non-clinical teams to deliver high-quality care services.

Key Responsibilities:
  • Conduct assessments of clients in their homes to identify their care needs and develop personalized care plans.
  • Monitor client progress, make adjustments to care plans as needed, and ensure that clients receive the best possible care.
  • Collaborate with medical practitioners and allied health professionals to coordinate care services and ensure seamless transitions between healthcare providers.
  • Document all interactions with clients, including care assessments, care plans, and progress updates, to maintain accurate records and ensure compliance with regulatory requirements.
Requirements:
  • Minimum Bachelors degree in Nursing - Current AHPRA registration
  • Current CPR / First Aid Certificate
  • Current and satisfactory Criminal Record Check
  • Current and satisfactory NDIS Screening Card and Working With Children Check
  • Current Australian Driver's Licence
  • Previous experience in the home care package, community care or health/aged care sector
