Job Summary

We are seeking an experienced Care Coordinator to provide person-centered care and support to clients in rural and remote regions of Australia.

Responsibilities
  • Providing care and support to clients with varying needs, including those with dementia and high care requirements.
  • Developing and implementing care plans tailored to individual client goals.
  • Collaborating with families, carers, and multidisciplinary teams to ensure holistic care delivery.
  • Maintaining accurate documentation in line with organisational and regulatory standards.
  • Promoting a safe and inclusive environment for clients and colleagues.
Requirements

To be successful in this role, you will need:

  • A Certificate III (or higher) in Aged Care, Home and Community Care, or Disability.
  • Qualifications and experience as a Care Coordinator, with a minimum of three years' experience working in community or residential aged care.
  • Demonstrated knowledge and/or recent experience in aged care, including dementia care and manual handling.
