Care Manager

We are seeking a highly skilled and compassionate Care Manager to join our team at MiCare. As a Care Manager, you will play a pivotal role in enhancing the quality of life for our elderly clients living in the community.
Your primary responsibility will be to ensure that our clients receive person-centred care and services that meet their unique needs and preferences. This will involve conducting initial assessments, care planning, ongoing review and monitoring of clients, as well as liaising closely with internal and external stakeholders to ensure seamless service delivery.
You will be responsible for initiating case and planning meetings, ensuring solid documentation, and following through on client health, care and service delivery. Additionally, you will develop individualised budgets for clients with Home Care Packages, ensuring spending compliance within the Home Care Package Guidelines.
We are looking for a proactive, reliable, and excellent communicator with a solid understanding of the Aged Care Quality Standards and Commonwealth Government community aged care programs. You will be able to work with others, be a participative and supportive team member, and meet compliance requirements of the role.
Qualifications for this role include a tertiary qualification in Allied Health, Nursing or Social Work. If you are passionate about person-centred care and have a commitment to enhancing the lives of our elderly clients, we encourage you to apply.
