Clinical Pharmacist

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at Epworth Hospital in Richmond. As a key member of our multidisciplinary team, you will play a vital role in providing exceptional care to patients, from admission to discharge.
Key Responsibilities- Prioritize patient needs and admission reviews from referring doctors
- Complete Best Possible Medication History for high-risk patients or as directed by ED doctors
- Conduct medication chart reviews and medication reconciliation
- Dispense and supply medication and pharmacy products
- Coordinate patient medication requirements for inpatient stays, day infusions, and discharge
- Deliver high-level clinical pharmacy services to patients in accordance with professional, legislative, and ethical standards
- Develop and maintain professional relationships with key stakeholders to ensure excellent customer service
- Support patients, carers, hospital staff, and associated employees
- Educate and advise staff and customers on health promotion, disease prevention, drug selection, and proper use of medicines
