Care Manager Role

We are seeking a dedicated and experienced Client Care Manager to join our team. The successful candidate will be responsible for delivering person-centred care, mentoring staff, and guiding behaviour support strategies.
Main Responsibilities- Tailor support to clients with complex needs by developing tailored care plans and providing individualised support.
- Lead behaviour support and crisis management strategies by working closely with multidisciplinary teams and ensuring effective communication and collaboration.
- Mentor and guide staff to build a high-performing team by coaching and supporting colleagues in their roles and responsibilities.
- Engage clients and families to align services with individual goals by actively listening to client needs and preferences and working collaboratively to achieve desired outcomes.
- Advocate for client rights and maintain accurate documentation by ensuring that all relevant information is recorded and reported in a timely manner.
- E nsure compliance with NDIS standards and policies by staying up-to-date with the latest regulatory requirements and best practices.
- Proven experience in disability support and a strong understanding of the NDIS and its principles.
- Strong communication and interpersonal skills, including the ability to work effectively with clients, families, and colleagues.
- Ability to mentor and model best practice, including coaching and supporting colleagues in their roles and responsibilities.
- Certificate III or IV in Disability Support (or equivalent) and current Working with Children's Check, NDIS Worker Screening, Police Check, First Aid, and Driver's Licence.
