Role detailsThe role is based in Ipswich and reports to the Facility Manager, with support from the Care Manager. The Clinical Care Coordinator & Nurse Mentor will supervise and support the care team and ensure residents receive high-quality clinical care. The role includes management and resourcing of care needs, maintaining up-to-date documentation and assessments in line with best-in-class clinical practices.
Key Responsibilities- Provision of clinical expertise and leadership of clinical and care service teams across the facility.
- Assist the Care Manager to lead care teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve best-practice outcomes.
- Manage all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility.
- Support documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
- Coordinate assessment, care planning and review processes within the facility.
- Support admission of new residents for optimal care assessments and initial consultations with residents and families.
- Ensure accurate information recording in accordance with organisational and legislative requirements (AN-ACC, resident records and staff time recording).
- Input and oversight into the facility team rosters and ensure optimal skill mix for care delivery.
- Manage and document clinical risk within the facility and drive continuous improvement.
- Innate empathy for residents and commitment to continually improving clinical care.
- Minimum 1 year experience as a Care Coordinator or similar leadership position.
- Current unrestricted Registered Nurse Div 1 with AHPRA.
- Postgraduate qualifications in aged care advantageous but not essential.
- Experience and knowledge of aged care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
- Ability to influence and develop clinical leadership across the clinical care team in an aged care environment.
- High computing skills to maintain documentation and reporting in line with requirements.
- Strong communication and interpersonal skills with a passion for leading clinical outcomes.
- Ability to manage multiple stakeholders and stay composed in emergencies and stressful situations.
- Have a valid NDIS Worker Screening Clearance (or ability to apply).
