**Homelessness Prevention Specialist**
We are seeking an experienced professional to join our team in Mandurah, WA. The successful candidate will have a strong background in case management and assessment processes.
**Key Responsibilities**
- Respond to referrals of individuals and/or families from internal and external services
- Undertake formal face-to-face assessments of clients, including all necessary paperwork using approved assessment tools
- Develop strong relationships with key external stakeholders to assist in the receipt of information and referral of clients
- Actively participate in local networks and forums to promote the program across all appropriate service providers in the region
- Undertake a range of case management duties to support the development of clients, including referrals and support letters, interaction with other service providers, appointment setting, and advocacy internally and externally
**Requirements**
- A Degree or Diploma in Social Welfare/Social Science or Community Services is highly desirable or a minimum of three years' experience in a similar role
- Have a comprehensive understanding of the issues impacting vulnerable and disadvantaged young people in the Peel Region
- Have experience working with parents and families of disengaged young people
- Demonstrated experience in providing vulnerable clients with a range of supports
- Sound knowledge of and relationship with referral sources, government, and non-government agencies
- Excellent communication skills, both written and verbal, and strong organisational skills
- Full driver licence with no restrictions
