Social Support Specialist

Job Description

We are seeking a dedicated individual to support people with disabilities in their daily lives. The successful candidate will provide personal care, assist with daily tasks, and facilitate access to community services.

Main Responsibilities:

  • Support participants with personal care, washing, cleaning, laundry, meal preparation, and shopping
  • Facilitate access to community services and facilities
  • Maintain accurate records of participant interactions and activities
  • Promote independence and quality of life for participants

Key Performance Indicators:

  • Present a professional image at all times
  • Maintain excellent standards of personal hygiene
  • Conduct yourself in a manner that aligns with organizational policies and procedures
  • Assist others when needed while demonstrating consideration for proper procedure

Workplace Safety:

  • Report accidents or near misses promptly using the incident reporting form
  • Complete thorough risk assessments prior to attending participant sites
  • Maintain a tidy environment free from hazards

Participant Engagement:

  • Tailor support to meet individual needs and preferences
  • Communicate effectively with participants, families, and caregivers
  • Display sensitivity to participant's cultures, languages, communication needs, and customs

Requirements:

  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Basic computer skills and proficiency in using software applications
