Care Advisor Role

Job Summary:

We are seeking a highly motivated and self-driven Care Advisor to join our team. This role is responsible for building relationships with key stakeholders and implementing a customer-focused culture.

The successful candidate will have a strong understanding of the National Disability Insurance Scheme (NDIS) and Home Care Packages (HCP). They will possess excellent communication and interpersonal skills, with the ability to connect with multiple stakeholders and think commercially.

In this role, you will be responsible for developing and maintaining effective relationships with clients, families, and carers. You will meet with clients to develop and refine their goals, provide information and support to monitor the use of their funds, and ensure accurate case noting and client reviews.

You will also be responsible for compliance with health policies and procedures to ensure a safe working environment and detailed, timely incident reporting.

Key Responsibilities:
  • Develop and maintain effective relationships with clients, families, and carers.
  • Meet with clients to develop and refine their goals.
  • Provide information and support to clients and their families/carers to monitor the use of their funds.
  • Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan.
  • Compliance with Health policies and procedures to ensure a safe working environment and detailed, timely incident reporting.
Requirements:Essential Skills and Qualifications:
  • Understanding of NDIS and Home Care Packages (HCP).
  • Qualifications in Nursing, Allied Health, or social science.
  • Previous case management experience.
  • Demonstrated knowledge of the Aged Care funding model.
Desirable Skills and Qualifications:
  • Exceptional customer service, negotiation, and influencing skills.
  • Excellent ability to connect with multiple stakeholders and think commercially.
  • Strong interpersonal skills and high levels of resilience.
  • Driver's license and access to own vehicle.
  • Professional verbal and written communication skills.
