Senior Caregiver Opportunity

Making a meaningful difference in people's lives is at the heart of this role. As a Senior Personal Carer, you'll have the chance to build lasting relationships and create a positive impact through everyday moments.

Key responsibilities include conducting comprehensive assessments, developing individualized care plans, and providing high-quality support based on clients' unique needs.

  • Conduct thorough initial screenings, creating tailored care plans for personal self-care, household tasks, social support, respite, transportation, and shopping assistance.
  • Ensure accurate client information is maintained in CRM & CMS systems, with up-to-date details and case notes.
  • Promote ongoing care discussions, planning, assessment, reviews, and goal setting in collaboration with the care team as required.
  • Engage with clients in their homes, supporting daily activities and delivering quality care according to individualized plans.
  • Participate in community-based recreational and social activities, fostering connections and well-being.
  • Provide transportation assistance for appointments and shopping.
Requirements:
  • Certificate III Aged Care, Individual Support, or equivalent - Essential.
  • Australian Driver's Licence (Green Provisional minimum) - Essential.
  • Roadworthy vehicle with Third Party Property insurance - Essential.
  • Proficiency in technology, including smartphones, tablets, and laptops.
  • Ability to meet relevant probity checks requirements.
