About the RoleWe are seeking an experienced Dietitian to work with various patients in residential aged care facilities and home care settings in the Geelong area. The successful candidate will have strong communication skills and ability to provide holistic care as part of a multidisciplinary team.
Main Responsibilities:
- Conduct comprehensive nutritional assessments for residents and clients in their own homes through home care packages/NDIS.
- Develop personalized nutrition plans and goals.
- Support dietary requirements for individuals with difficulty swallowing and chewing, bowel and intestinal issues, and other complex conditions.
- Provide weight management, poor appetite, and dysphagia interventions.
- Manage food intolerances and allergies.
- Deliver education on diabetes management.
- Educate clients and their families to optimize their ability to live healthy lives within the community.
- Work collaboratively with allied health professionals to develop a comprehensive care plan.
- Liaise with service providers to ensure effective communication.
