Support Practitioner

This is an exciting opportunity to join Churches of Christ in Queensland as a Support Practitioner - Identified. As a key member of our Children, Youth and Families team, you will play a vital role in providing high-level support to children, young people, foster and kinship carers, and families of Aboriginal and Torres Strait Islander descent.
Key Responsibilities- Screening and Intake: Undertake thorough screening, intake, and placement partnering processes for children and young people with foster and kinship care families.
- Crisis Intervention: Utilize a range of assessment, crisis, and short-term intervention skills and therapeutic approaches to clients, reporting issues to management as necessary.
- Program Development: Contribute to the development, implementation, and evaluation of programs and care arrangements to meet individual needs.
- Parenting Programs: Facilitate the delivery of evidence-based parenting programs to parents and carers, utilizing appropriate modelling techniques to address identified needs and vulnerabilities.
- Carer Support: Coordinate recruitment of and provide ongoing support to carers, ensuring they have the necessary resources and guidance to provide high-quality care.
- Assessment and Reporting: Generate comprehensive assessment and/or reports regarding the initial and ongoing suitability of carers and/or young people, facilitating the foster care approval process as required.
- Case Management: Negotiate, coordinate, and implement case management and planning, supporting the integration of joint case management processes and maintaining detailed and accurate client records.
- Direct Support Services: Provide direct in-home and flexible support services, including after-hours and 'on-call' services on a rotational basis as required, negotiating intervention plans when necessary and providing advice and referrals as needed.

Requirements- Qualifications: A degree qualification in social work, psychology, social sciences, or a related discipline, or an equivalent level of expertise attained through previous appointments, service, and/or study.
- Experience: One year's experience working with children, young people, and families in a child protection or community service context.
- Knowledge: Applied knowledge and understanding of child protection and out-of-home care frameworks, including knowledge of the Child Protection Act 1999.
- Skills: Demonstrated ability to complete assessments for care, experience working with First Nations people, and a sound understanding of child development and trauma theory to inform assessment and decision-making.
- Driver's License: A current driver's license with a P2 minimum.
