Chris Barnard's Nursing Agency has a proven track record of success in the Aged Care sector, with over 14 years of experience in Victoria and 2+ years in Hobart. We are now seeking a highly skilled and motivated individual to join our team as a founding member of our Home Care arm in Hobart.

About the Role

We are looking for an experienced Home Care Case Manager who is passionate about delivering high-quality care services to older Australians. As a founding member of our Home Care team, you will have the opportunity to shape the direction of our business and contribute to the growth and development of our team.

Key Responsibilities
  • Sourcing clients with a proven track record in the HCP
  • Developing and implementing care plans that meet the needs of clients
  • Providing exceptional customer service and ensuring client satisfaction
  • Working collaboratively with the team to achieve business objectives
  • Staying up-to-date with industry developments and best practices
Requirements
  • 5+ years of experience in the Aged Care sector
  • Proven track record in sourcing clients with a HCP
  • Excellent communication and interpersonal skills
  • Ability to work in a team environment and contribute to business growth
  • Knowledge of the Aged Care Standards and ability to apply person-centered care services
