Aged Care Pharmacy Professional

We are seeking a highly skilled Pharmacist to join our multidisciplinary healthcare team, providing collaborative care for residents in our residential aged care facilities.
The ideal candidate will develop strong working relationships with the healthcare team and provide expertise in medication management, ensuring continuity of care for our residents.
Key responsibilities include:
- Providing expert guidance on medication management and monitoring resident's medication regimens
- Supporting medication management during transitions of care and ensuring seamless patient transfers
- Conducting quality use of medicines activities, such as medicine use evaluations and implementing changes identified through audits
- Responding promptly to medication-related inquiries from residents, families, and carers
To succeed in this role, you will possess:
- Registration as a pharmacist with a valid Australian Health Practitioner Regulatory Agency (AHPRA) number
- Completion of an Accredited Program for Pharmacists in Aged Care training program
