Nurse Practitioner

Catholic Healthcare is seeking a dedicated and compassionate Registered Nurse to join our Home and Community Team. As a key member of our team, you will provide high-quality nursing care to our clients in their homes, promoting their health, well-being, and independence.
About the Role- Provide comprehensive nursing assessments, care, and evaluations to clients with complex health needs.
- Develop and implement individualized care plans in collaboration with clients, carers, and healthcare professionals.
- Liaise with multidisciplinary teams to ensure holistic care and support for clients.
- Ensure accurate and timely documentation of client care and progress.
- A current AHPRA registration and a Bachelor of Nursing or similar qualification.
- Minimum 3 years post-graduate nursing experience, with a focus on clinical assessments and care planning.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, carers, and healthcare professionals.
- Sound computer skills and the ability to learn new systems.
- A valid driver's license and a reliable vehicle.
