Aged Care Professional Opportunity

As a Clinical Aged Care Specialist, you will play a pivotal role in identifying the needs of older adults and connecting them with services that promote independent living with the highest quality of life.

  • Conduct Comprehensive Assessments: Perform thorough Aged Care Assessments to identify the needs of older adults, taking into account their physical, emotional, and social well-being.
  • Facilitate Service Coordination: Coordinate referrals to essential services supporting independent living, ensuring seamless transitions and optimal outcomes.
  • Promote Functionality and Independence: Actively promote the retention and maximization of function and capability in daily living, empowering older adults to maintain their autonomy.

Your Qualifications
  • A minimum of 1 year's experience in aged care, including client assessments and/or service delivery, with a strong focus on building relationships and delivering results.
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work), with a solid understanding of ageing theories and models.
  • Current unrestricted General Registration with AHPRA or relevant professional association, ensuring you meet the required standards and guidelines.
  • Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and client management systems, enhancing your productivity and efficiency.
