Job Title: Reablement Therapist Specialist
Role Overview:We are seeking an experienced Occupational Therapist to join our dynamic Reablement Team in Wiltshire. As a key member of our team, you will play a pivotal role in empowering individuals to rebuild their lives after hospitalization.The ideal candidate will have a passion for transforming lives and restoring confidence through personalized reablement plans, collaboration with healthcare professionals, and a commitment to excellence.,
Key Responsibilities:1. Conduct comprehensive strengths-based assessments to identify individual needs and develop tailored reablement plans.
2. Collaborate with healthcare professionals and social workers to achieve the best outcomes for service users.
3. Manage a diverse caseload, prioritizing complex cases and providing supervision and support as needed.
4. Act as an expert advisor on bespoke equipment provision, evaluating reasoning, and making decisions to increase independence and reduce risk.
5. Promote a culture of inclusion, dignity, trust, and respect within the team and across services.

Requirements:- Over three years of post-qualification experience in Occupational Therapy.
- Full UK driver's license.
- Understanding of occupational therapy laws and regulations.
