Job Summary

We are seeking a dedicated and compassionate professional to fill the role of Case Manager. This position involves providing high-quality psychosocial support services to clients, working collaboratively with colleagues to achieve client outcomes, and maintaining accurate records and reports.

Key Responsibilities
  • Deliver tailored support plans that cater to the unique needs and goals of each client.
  • Conduct thorough assessments to ensure effective service delivery.
  • Maintain open communication channels with clients, families, and multidisciplinary teams.
  • Collaborate with colleagues to develop and implement comprehensive support strategies.
  • Ensure accurate and timely documentation of client interactions and progress.
Required Qualifications
  • Holds a degree in Social Work or a related field.
  • Prior experience in case management or social work is highly regarded.
  • Eligibility for membership with the Australian Association of Social Workers (AASW).
  • Current certification as per AASW requirements.
