Home Care Packages Case Manager

We are seeking a skilled and compassionate Home Care Packages Case Manager to join our team. As a key member of our home care services division, you will work closely with clients, their families, and other healthcare professionals to deliver high-quality, client-centered care.

This is a challenging yet rewarding role that requires strong communication, organizational, and interpersonal skills. You will be responsible for developing and implementing individualized care plans, conducting assessments, and coordinating care services to meet the unique needs of each client.

Key responsibilities include:

  • Conducting comprehensive assessments of clients' needs and goals
  • Developing and reviewing individualized care plans in collaboration with clients and their families
  • Coordinating with internal and external service providers to deliver holistic care services
  • Maintaining accurate records and reporting as required

To succeed in this role, you will need:

  • A relevant tertiary qualification in a human services discipline, such as social work or nursing
  • Proven experience in case management or a related field
  • Strong knowledge of community care principles and practices
  • Excellent communication and interpersonal skills
