Job Description:

We are seeking a dedicated Clinical Care Coordinator to join our aged care team. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality clinical care to residents, collaborating with healthcare professionals to create personalized care plans, and supporting residents and their families with compassion.

Key Responsibilities:

  • Assessing Patients and Forming Personalized Care Plans: You will conduct thorough assessments of patients to identify their needs and develop tailored care plans that promote optimal health and well-being.
  • Delivering Personalized Care: You will deliver compassionate and person-centered care to residents, respecting their individual needs and preferences.
  • Precise Preparation and Administration of Medication and Treatments: You will accurately prepare and administer medication and treatments as prescribed by healthcare professionals.
  • Careful Monitoring of Patient Well-being: You will closely monitor patients' conditions and address any concerns or issues promptly.
  • Collaboration with Multidisciplinary Team: You will work effectively with healthcare professionals to ensure seamless delivery of care services.
  • Accurate Maintenance of Documentation: You will maintain accurate records of patient information, ensuring compliance with regulatory requirements.

Required Skills and Qualifications:

  • Current AHPRA Registration: You must be registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Strong Communication and Interpersonal Skills: You should possess excellent communication and interpersonal skills, enabling you to build strong relationships with residents, families, and healthcare professionals.
  • Excellent Clinical Knowledge and Abilities: You should have advanced clinical knowledge and skills, allowing you to provide high-quality care to residents.
  • Empathetic and Compassionate Approaches: You should demonstrate empathy and compassion when interacting with residents and their families.
  • Ability to Collaborate with Healthcare Professionals: You should be able to work effectively with healthcare professionals to achieve common goals.
  • Prior Experience Essential: You should have prior experience working in aged care or a related field.
