**Clinical Care Coordinator Role Overview**

We are seeking an experienced and passionate Clinical Care Coordinator to join our residential aged care facility in Reservoir, Victoria. This is an excellent opportunity for a registered nurse or clinical care coordinator looking to expand their career in a dynamic and supportive environment.

About the Role:

  • Supervise and coordinate clinical care services to ensure high-quality care and quality of life for our residents.
  • Lead, mentor, and support the nursing team, managing schedules, conducting performance reviews, and fostering a positive work environment.
  • Develop and review individualized care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
  • Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
  • Ensure accurate and timely documentation of care plans, incidents, and resident records.

Key Requirements:

  • Prior experience as a Clinical Care Coordinator (or similar position) preferred, but not essential.
  • You must be an experienced Registered Nurse with a current AHPRA registration with the aspiration to expand your career or an experienced CCC looking for a new opportunity.
  • Strong clinical knowledge, leadership, organisational, and communicative skills and the ability to lead, manage and motivate a clinical team effectively.
  • Excellent knowledge and experience of aged care accreditation, regulations, standards, and best practices.

Estimated Salary:** $90,000 - $110,000 per annum (based on location and experience)
