About the Role

We are seeking a skilled and compassionate Social Worker to provide casual relief support to our multidisciplinary team across various programs, including inpatient, outpatient, and home-based palliative care services.

As a Casual Social Worker, you will be responsible for providing psychosocial assessments, interventions, and support to patients and their families, including assistance with financial and accommodation issues, adjustment to diagnosis, advance care planning, and referrals to community agencies.

The ideal candidate will have experience in hospital social work, palliative care, progressive neurology, and/or aged care, with strong knowledge of systems and supports that help people live well with advanced illness and/or aging.

We offer a flexible casual position with variable hours and rosters, with opportunities to work across a range of areas, including inpatient and home-based palliative care, inpatient and outpatient progressive neurology, Carer Support Program, and Commonwealth Home Support Program.

What You Bring
  • Experience in hospital social work, palliative care, progressive neurology, and/or aged care
  • Strong knowledge of systems and supports that help people live well with advanced illness and/or aging
  • Ability to work autonomously and as part of a team
  • Tertiary qualification in social work
  • Eligible for registration with the Australian Association of Social Work (AASW)
