About the Role

AAPM Futures is seeking a highly motivated and experienced Facilitator, Career Coach to join our team. As a key member of our team, you will be responsible for delivering highly individualized employment supports to NDIS Participants.

Key Responsibilities

  • Provide support and mentorship to participants that encourages skills development and increases their capacity for work
  • Deliver person-centered supports to participants transitioning from school to work
  • Provide on-site support once employment has commenced and assist with integration
  • Travel as required for service delivery and support of participants

Requirements

  • Eligibility to work in Australia
  • A current driver's license and a comprehensively insured vehicle
  • Current First Aid and CPR certification
  • Willingness to complete a Criminal History Check
  • Able to pass a Working with Children Check
  • Able to pass a NDIS Worker Screening Check
  • Able to pass and complete an NDIS Worker Orientation Module prior to commencement
