We are seeking a skilled and compassionate healthcare professional to join our team in Toowoomba, Queensland.
The successful candidate will work collaboratively in a primary care setting, delivering high-quality patient-centered care and promoting overall health and wellbeing within the community.
About the Role- Patient Assessment and Care Planning: Provide comprehensive assessment, triage, and care planning services for patients.
- Clinical Expertise: Deliver therapeutic care and treatment plans that align with best practices and evidence-based guidelines.
- Clinical Data Management: Accurately manage clinical data, communicate effectively with other services, and promote seamless care coordination.
- Patient Education and Support: Develop and implement education and self-management support strategies to enhance patient outcomes.
- Infection Control and Advocacy: Advocate for patients and promote infection control practices to ensure a safe and healthy environment.
- Registration and Certification: Hold current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
- Experience and Skills: Possess minimum 2 years' experience as a Registered Nurse, preferably in a General Practice (GP) setting, with strong clinical skills and knowledge of evidence-based nursing practices.
- Communication and Teamwork: Demonstrate excellent communication and interpersonal skills, with the ability to work autonomously and collaboratively as part of a multidisciplinary team.
