Primary Care Clinician

We are seeking an experienced Primary Care Clinician to drive localised provision of healthcare services, reducing the need for hospital access.

The Role
  1. Responsible for telephone triaging and assessing the need for face-to-face appointments.
  2. Act as an autonomous clinician and take responsibility for assessment, treatment plans, and/or referrals to specialist services as required.
  3. Be compliant with professional conduct and national guidelines, adhering to their guidance.
  4. Take a comprehensive patient history, perform physical examinations, and use expert knowledge and clinical judgment to identify potential diagnoses.
  5. Refer patients for investigations and/or specialist care where necessary.
  6. Independently decide on and carry out treatment, including prescribing medicines.
  7. Make onward referrals to other services as appropriate.
  8. Ensure continuity of care.
  9. Work independently and as part of a local and wider healthcare team.
  10. Provide leadership and ensure that each patient's treatment and care is based on best practice.
