About the Role

Cocoon Group Services is a dynamic and innovative organization seeking a dedicated Disability Support Worker to join our team in Logan Reserve, QLD.

As a Disability Support Worker, you will play a vital role in supporting participants with complex care needs, personal care, and high physical needs in a Supported Independent Living (SIL) or SDA setting.

Key Responsibilities
  • Provide personal support and empower participants to reach their goals and maintain independence.
  • Assist with daily living activities, including personal care, meal preparation, and household management.
  • Support participants with complex care needs, including those with mental health conditions, intellectual disabilities, and behaviors of concern.
  • Develop and implement individualized support plans to promote participant independence and well-being.
  • Collaborate with the multidisciplinary team to ensure comprehensive care and support.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • Certificate III in Individual Support (Disability, Aged Care, or equivalent).
  • Current CPR, First Aid, Manual Handling, and Medication Administration training.
  • Current Driver's License.
  • NDIS Worker Screening Check and Working With Children Check (or willing to obtain).
