Job Title:

An Intensive Education Case Manager will play a pivotal role in the development and coordination of individualised case management plans in collaboration with key stakeholders.

About the Role:
  • This position involves providing intensive case management that may require educational and behaviour intervention for students at risk of disengagement from education, in accordance with legislative, policy and procedural requirements.
  • The ideal candidate will demonstrate a proven ability to establish and sustain effective partnerships with young people, parents/carers, school principals and other stakeholders in order to deliver tailored, intensive case management in alignment with legislative, policy, and procedural requirements to support students at risk of disengaging, or who have disengaged, from education.
Key Responsibilities:
  • Develop and coordinate individualised case management plans in collaboration with key stakeholders, including behaviour support plans, risk assessments, education and transition plans and referrals to appropriate services.
  • Provide intensive case management that requires educational and behaviour intervention for students at risk of disengagement from education, adhering to legislative, policy and procedural requirements.
  • Analyse information and data to gain an understanding of the young person's strengths and provide strategic advice on identified concerns to inform effective case-management, integrated support planning and inform service enhancements.
Skills and Qualifications:
  • Ability to develop and maintain effective partnerships with young people, parents/carers, school principals and other stakeholders.
  • Strong analytical skills to gather and interpret information and data relevant to the young person's case.
  • Proven ability to work effectively in a team environment to deliver intensive case management and support services.
