Home Support Specialist

The Home Support Specialist plays a vital role in assisting older individuals to remain independent and engaged within their communities.

Key Responsibilities:
- Conduct thorough assessments of clients referred via the My Aged Care Gateway using the National Screening Assessment Form.
- Match and refer clients to appropriate services based on their assessed needs.
- Provide ongoing support to clients who require coordination of service linkage and re-ablement assistance.
Requirements:
- Tertiary qualification in Ageing Support, Individual Support (Ageing), Community Services Work, or related field.
- Current Australian driver's license.
- Australian working rights.
