Clinical Care Specialist

We are seeking a skilled professional to provide high-quality clinical care to our residents within a remote aged care facility.
This role is ideally suited to someone who enjoys working and supporting elderly care in isolated locations. The successful candidate will have a compassionate nature and be able to work independently with minimal supervision.
Key Responsibilities:- To develop, implement, monitor and review individualized care plans that cater to the unique health needs and preferences of each resident.
- Report any changes or concerns in a resident's condition promptly.
- To deliver nursing care within the scope of established clinical practices.
- Ensure that resident care is thoroughly assessed, planned, implemented, and evaluated according to our comprehensive model of care.
The ideal candidate will have previous experience as a Registered Nurse, preferably in a geriatric or aged care environment. They will also possess excellent communication skills and be able to collaborate effectively with a multidisciplinary care team.
