About This Role

The Team Lead oversees a multidisciplinary team within Home and Community Care, ensuring optimal performance and service delivery.

Key Responsibilities
  • Spearhead the team's performance, guaranteeing adherence to goals and service standards.
  • Manage day-to-day operations, including worker absences, client cancellations, and roster changes, for efficient service delivery.
  • Collaborate with Workforce Management and Sales teams to optimize rostering and smooth client onboarding processes.
  • Conduct one-on-one supervision meetings, facilitate team gatherings, and oversee training compliance and competency assessments.
  • Provide leadership in WH&S practices, maintaining a safe working environment and responding to incidents as needed.
  • Work with the Care Excellence Lead to enhance clinical capability, identify gaps, and implement coaching or supervision plans.
  • Analyze and interpret service data and audits to drive improvements and compliance actions.
  • Serve as an escalation point for team and client concerns, resolving conflicts, managing feedback, and promoting client-centred care.
Requirements & Qualifications
  • Qualifications in Aged Care, Community Services, Nursing, or a related health field.
  • Experience leading teams in home care or community services environments.
  • Strong understanding of person-centred service delivery, WH&S, and rostering practices.
  • Demonstrated ability to coach, support, and supervise staff.
  • Excellent communication and conflict resolution skills.
  • Proficiency in using rostering or client management systems.
  • Familiarity with aged care standards, relevant legislation, and continuous improvement frameworks.
