We are seeking a dedicated and compassionate individual to join our team as a Care Partner. As a Care Partner, you will play a vital role in enhancing the lives of our seniors by providing personalized care and support.

Your primary responsibility will be to conduct thorough client assessments, identifying their unique needs and goals. This information will inform the development of customized care plans that cater to their specific requirements.

You will work closely with clients, their families, and our support team to ensure seamless coordination of services. Your attention to detail and problem-solving skills will enable you to identify opportunities for enhanced independence and well-being.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build trust and rapport with clients from diverse backgrounds. A Certificate III in Community Services or equivalent experience is essential, along with a current SA Driver's Licence and reliable vehicle.
