About the Job

This role is an exciting opportunity to be part of a dynamic team at HammondCare, working on a Dementia Centre initiative that aims to support individuals living with dementia.

As a Family Liaison Officer, you will be responsible for providing compassionate pre- and post-placement support and counselling for families and care teams, assessing clients' needs, abilities, and behaviours to inform personalised care plans.

The Role

  • You will collect valuable data to ensure a seamless transition for clients with dementia from acute care to residential or home settings.
  • You will explore and understand opportunities and challenges around the transition and placement of someone living with dementia.

Key Responsibilities

  1. Assess clients' needs, abilities or behaviour using various methods.
  2. Build rapport and a working relationship with families/representatives participating in the program.
  3. Foster collaboration between families/representatives, Hospital Staff and aged care staff during the pre and post placement phase.
  4. Support families and manage expectations around placement of someone living with dementia.
  5. Participate in family and case conferences.
  6. Provide emotional support and practical advice for families in relation to obtaining a placement and supporting the person living with dementia to transition out of hospital.
  7. Collect data sets to aid in better understanding what is needed for successful placement of someone living with dementia.

Requirements

  • Social Worker or registered Psychologist experience is essential.
  • Strong experience and knowledge in dementia care and mental health for older persons is required.
  • High level communication and interpersonal skills to lead and influence are necessary.
  • Excellent problem solving and decision-making skills are required.
  • Excellent written and verbal communication skills are essential.
  • Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action is necessary.
  • A current Australian drivers licence and willingness to travel intrastate and interstate is also required.
