Job Summary

We are seeking a skilled Clinical Program Coordinator to join our team. In this key position, you will play a crucial role in coordinating the delivery of service to clients with complex care needs.

About the Role

This is a challenging and rewarding role that requires strong clinical knowledge, excellent communication skills, and the ability to work autonomously. As a Clinical Program Coordinator, you will be responsible for developing and implementing client care plans, providing clinical guidance, and supporting clients and network staff.

Key Responsibilities:
  • Client Care Planning: Develop and review client care plans, ensuring they meet individual needs and align with organizational goals.
  • Clinical Guidance: Provide expert clinical advice and support to clients and network staff, promoting high-quality care and patient outcomes.
  • Team Collaboration: Work closely with allied health professionals, support coordinators, and external stakeholders to ensure seamless coordination of client care.
  • Stock Management: Oversee stock orders and review client progress notes, maintaining accurate records and ensuring timely interventions.
Requirements:
  • AHPRA registration as a Registered Nurse (with nil restrictions)
  • Strong passion for health education, skills in health assessment, planning, and goal setting
  • Excellent communication, organisational, and computer skills
  • Ability to work remotely and autonomously when required
