Disability Care Specialist Role:
We seek a skilled professional to work with individuals living with disabilities, providing person-centered care solutions and supporting clients in achieving their goals.
Key Responsibilities:
- Delivering high-quality care services and identifying client goals and objectives.
- Supporting individuals in formulating their goals and aspirations prior to planning meetings.
- Participating in team meetings and case conferences to ensure timely and accurate record keeping.
- Collaborating with clients and stakeholders in developing care plans and agreements.
- Providing tailored services that meet individual requirements.
- Monitoring and reviewing services to ensure quality improvement measures are met.
- Responding to client feedback and managing outcomes.

Required Skills and Qualifications:
- Qualifications in Community Services or relevant demonstrated experience.
- Excellent administrative, time management, and organizational skills.
- Experience working within the Disability sector, including knowledge of NDIS standards.
- Understanding of up-to-date Disability related legislation and processes.
- Ability to work collaboratively with internal and external stakeholders.
- Autonomy and initiative in problem-solving.
- High degree of professionalism, optimism, and resilience.
- Excellent interpersonal and communication skills.
