Care Coordinator

We provide early intervention, rehabilitation, therapy, assistive technology and disability services to over 6,000 people living with a disability. Our primary purpose is to enable individuals with a disability and their families to participate in the community to their full potential.
Role: Disability Support Professional – Murray Bridge (Part-time)This part-time role is based at Murray Bridge and involves providing in-home supports as part of a coordinated care approach. Shifts are scheduled during weekday mornings and afternoons. You will use your knowledge and experience supporting individuals with physical and intellectual disabilities to promote independence, community participation, and meaningful relationships in a safe and supportive environment.
In this role you will interpret and respond to an individual's communication needs, health care plan, and mealtime guidelines, administer medication, and document observations. Your contribution will help an individual reach their full potential and independence goals.
Responsibilities- Support planning and provision of a client-centred service
- Provide in-home supports as part of a coordinated care approach
- Interpret and act on individual health care plans, communication needs, and mealtime guidelines
- Identify and respond to health concerns and accurately document observations
- Administer prescribed medication and maintain accurate records
- Engage with clients, families, and colleagues with excellent verbal and written communication
- Maintain high attention to detail and adherence to safe work practices
- Promote our mission in the community
- A current NDIS Worker Check and Working with Children Check
- Certificate III in Individual Support, Disability, or Community Services
- Child Safe Environments training
- Completion of the NDIS Worker Orientation Module: Quality, Safety and You
- Competency-based training in Assist Clients with Medication
- Competency-based Manual Handling training
- A current valid Driver's Licence

Remuneration and BenefitsThe total remuneration package includes base salary, annual leave loading and statutory superannuation contribution. Salary packaging options are available up to $15,900 to increase take-home pay, with additional benefits such as meal and entertainment expenses and private novated leasing.
