Senior Care Manager Position

We are seeking a skilled Senior Care Manager to join our team, responsible for delivering personalized support plans for residents according to their home care package or fee-for-service requirements.

The ideal candidate will have strong partnership and communication skills, as well as the ability to understand and manage complex care plans and clinical assessments.

Key Responsibilities:
  • Collaborate with Care Managers to develop holistic support plans that cater to individual needs
  • Analyze and manage care plans, ensuring alignment with resident goals and objectives
  • Maintain current registration with AHPRA (Division 1) and obtain necessary certifications
  • Ensure compliance with regulatory requirements and standards
  • Promote a culture of safety and quality in all aspects of care delivery
