About the Role

This is a leadership position within our Home Care team where you'll be supporting both our clients and the care workers who assist them day to day.

As a leader in this role, you'll coordinate services, manage care plans, respond to client needs and provide guidance and support to your team members making sure they feel confident, equipped and valued in their roles.

This is a varied role where no two days are the same. You might be mentoring staff, reviewing services, solving rostering puzzles, or working with families and health providers to keep things running smoothly.

This role offers the rare opportunity to build genuine relationships with your clients while delivering meaningful care.

Key Responsibilities
  • Planning and coordinating care services for optimal outcomes.
  • Servicing as a point of contact and mentor for Home Care Workers providing them with the necessary support and guidance.
  • Collaborating with healthcare professionals, allied health and families to ensure seamless service delivery.
  • Evaluating and ensuring services align with best practice and meet client requirements.
  • Bonding with clients and their caregivers through strong interpersonal skills and relationship building.
Our Ideal Candidate
  • Possesses a Certificate III in Individual Support (Ageing) or similar qualification.
  • Cert IV desirable or willing to obtain it within a specified timeframe.
  • A minimum of 1 year of experience leading a care or support team.
  • Current driver's license and insured vehicle.
  • First Aid and CPR certifications.
  • Familiarity with Home Care Packages.
