Job Title: Healthcare Professionals in Patient Safety and Quality

">Responsibilities:
">- Lead and manage staff by fostering a culture of patient safety and quality in the delivery of healthcare services.
Assess patients' needs and develop appropriate management plans in consultation with consultants.
">Provide ongoing clinical care to patients, including gathering input from multidisciplinary teams and communicating assessment findings and proposed management plans to patients and carers.
">Continuously monitor and review patient progress and adjust management plans as necessary.
">Collaborate with other healthcare providers and carers to ensure seamless care post-hospital stay.
