About the Role:

This position plays a vital role in a multi-disciplinary team to deliver social work assessments and interventions for patients across various healthcare sites. The focus is on developing complex care plans and addressing psycho-social barriers to patient discharge or transfers of care.

The successful candidate will provide support with accessing external social services, especially those focused on welfare. They will also establish strong connections with local transport agencies and community transport providers, as well as funeral homes.

Service delivery will primarily be conducted through virtual platforms to ensure equitable access to the service for all non-base hospital facilities.

  • Conduct social work assessments and interventions for patients across healthcare sites

Required Skills and Qualifications:

The ideal candidate will hold a tertiary qualification in Social Work and possess eligibility for membership of the Australian Association of Social Workers (AASW). They must also have excellent written, verbal, interpersonal and negotiation skills, as well as high-level problem-solving and service evaluation skills.
