Clinical Nurse Specialist Grade 2

This is a temporary part-time position of 12 hours per week for leave relief until 30 March 2025, with the possibility of extension. The successful candidate will provide a multi-disciplinary approach to the screening, assessment, and care planning of older people (generally aged over 70) admitted to the Emergency Departments to ensure appropriate and coordinated assessment and care for the designated group in the Emergency Department.
Key Responsibilities- Provide advanced skills in the assessment and care management of older people with complex needs, including cognitive and behavioural assessment.
- Consult with the treating ED doctor, GPs, allied health, nursing, acute geriatric service, and community services to ensure comprehensive care.
- Develop and implement individualized care plans to meet the unique needs of older patients.
- Collaborate with a multi-disciplinary team to ensure seamless care coordination and communication.
- Utilize electronic information systems to collect and manage patient data.
- Prioritize and manage own caseload to ensure timely and effective care delivery.
- Registration with the NSW Nurse's Registration Board.
- Relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification.
- Demonstrated high-level ability to communicate clearly and succinctly both verbally and in writing.
- Proven ability to work effectively and collaboratively within a multi-disciplinary team.
- Computer literacy with reference to data collection, email, and word processing skills.
- Current NSW Class C driver's licence.
