Key Responsibilities:
  • Empowerment through Coordination: Assist participants in developing the skills and confidence needed to effectively manage and coordinate their supports.
  • Capacity Building: Support participants in building their capacity to make informed decisions and exercise control over their lives, ensuring they have the resources to live independently and meaningfully engage with their community.
  • Person-Centred Practice: Adhere to a person-centred approach that prioritizes the participant's autonomy, ensuring that all supports are aligned with the goals and outcomes specified in their approved plan, within a safe and supportive environment.
  • Implementation of Supports: Facilitate the implementation of the participant's plan by coordinating a range of supports, including funded services, mainstream and community resources, and informal supports, ensuring these are driven by the participant's individual needs and aspirations.
  • Operate within the NDIS framework: Ensure that all services are aligned with the participant's approved plan and that coordination efforts are centered around the participant's goals.
  • Collaboration: Work with participants, their families, and support networks to ensure that all services and supports are integrated, cohesive, and reflective of the participant's desires and needs.

Requirements:
  • Current NDIS Worker Screening Clearance
  • NDIS Worker Orientation Module Certificate
  • Working with Children Check
  • Diploma Community Services or equivalent experience
  • Current First Aid including current CPR
  • Current driver's license
  • Victorian Police Check
  • Infection Control Certificate
  • Knowledge of Microsoft Office Suite, with a proficiency in Word and Outlook
  • Experience in the disability sector
  • Demonstrated high level of verbal communication and interpersonal skills
  • Positive, empathetic and willing to learn new skills
  • Adaptable to an ever-changing disability landscape
  • Time management and autonomous working skills
