Key Responsibilities- Provides information about Home Care Packages and related rights and budget management to care recipients and their family members.
- Takes an active role in client case conferencing and coordinates meetings with key stakeholders as needed.
- Experience working in aged care, including understanding of issues affecting older people and those with disabilities.
- Familiarity with aged care standards and best practice in community settings.
- Strong communication, problem-solving, and influencing skills.
