Job Title: Community Health Advocate

About the Role:

This is a key position in our multidisciplinary team responsible for delivering healthcare services to marginalized and vulnerable adults in Suffolk.

Main Responsibilities:
  • Manage client cases, assess needs and risks, and deliver outreach services to hard-to-reach groups.
  • Provide holistic support to vulnerable clients, including proactively establishing contact and conducting thorough assessments of care requirements.
  • Develop, implement, and evaluate programs of care, setting standards and collaborating with other organizations as necessary.
Collaboration and Partnerships:
  • Liaise and work jointly with partners, particularly services targeting clients from asylum seeker and refugee, black and minority ethnic, ex-offender, gypsy and traveler, homeless, and migrant worker groups.
Skill Requirements:
  • Registered nurse or equivalent qualification
  • Proven experience working with vulnerable populations, including those with mental health issues
  • Strong communication and interpersonal skills
  • Able to work independently and as part of a cohesive team
  • Ability to manage complex caseloads and prioritize tasks effectively
