Job DescriptionThe Housing Support Professional plays a vital role in providing direct support to clients, helping them achieve their goals and overcome challenges. The successful candidate will work closely with clients to identify their needs, develop personalized plans, and provide ongoing support and guidance.
Key responsibilities include:
- Conducting client assessments and developing support plans
- Providing one-on-one counseling and coaching to help clients achieve their goals
- Assisting clients with daily tasks and activities, such as meal preparation and household chores
- Linking clients with community resources and services
- Documenting client progress and outcomes
The ideal candidate will possess excellent communication and interpersonal skills, be able to work independently and as part of a team, and have a strong commitment to supporting vulnerable populations.
