We are currently seeking a Mental Health Case Coordinator to join our team of experienced mental health professionals.

About the Role

This is a rewarding opportunity for a skilled Registered Psychiatric Nurse to work collaboratively with multidisciplinary teams to deliver exceptional patient care.

  • As a Mental Health Case Coordinator, you will work closely with consumers, carers, and multidisciplinary teams to develop and implement individualized care plans.
  • You will also be responsible for coordinating case management activities, ensuring seamless transitions between services and promoting a culture of recovery-oriented practice.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with consumers, carers, and healthcare professionals.

Key Responsibilities
  1. Develop and implement individualized care plans for consumers
  2. Coordinate case management activities to ensure seamless transitions between services

Requirements
  • Current registration as a Registered Psychiatric Nurse (Division 1 or 3)
  • Diploma or degree in nursing from a recognized institution
  • Minimum 2 years post-qualification experience in mental health
  • A valid Nationally Coordinated Criminal History Check
