Job Title: Aboriginal Health Rural Generalist Transitional Nurse Practitioner

About the Role

We are seeking a skilled and dedicated Nurse Practitioner/Transitional Nurse Practitioner to join our team. As a site-specific model, you will provide high levels of clinically focused care across a range of services including emergency, inpatient, chronic and complex care, and primary care.

  • Provide holistic care to patients with acute and chronic presentations across the lifespan to address patient and community needs.
  • Make referrals to facilitate integrated care across disciplines or teams and contribute to clinical excellence through research, teaching, consulting, and providing leadership to the nursing profession.
  • Work autonomously and collaboratively in an advanced and extended nursing clinical role, meeting the clinical demand within your defined and agreed scope of practice and formulary.
  • Develop expertise in chronic and complex health conditions and support the development of best practice emergency/inpatient care plans and coordination of discharge plans for identified high-risk patients.
  • Create culturally safe partnerships to address priority population groups within the community, including Aboriginal and Torres Strait Islander people, people living with a disability, people experiencing homelessness or on low incomes, older people, and culturally and linguistically diverse groups.

You will have the opportunity to work with internal and external community agencies and services relevant to patient conditions.

The position is grounded in the nursing profession's values, knowledge, theories, and practices, providing innovative and flexible healthcare delivery that complements other healthcare providers.

About You

This vacancy is eligible for a NSW Health Rural Health Incentive package of up to $5,000 per year in addition to award entitlements (paid on a pro-rata basis for part-time employees) and/or accommodation assistance when requested (conditions apply).
