Advanced Rural Nursing Practitioner Opportunity

The role of Advanced Rural Nursing Practitioner/Transitional Nurse Practitioner is a site-specific model providing high levels of clinically focused care across various services, including emergency, inpatient, chronic and complex care, and primary care.

  • Deliver comprehensive patient care addressing diverse acute and chronic presentations throughout the lifespan to meet individual and community needs.
  • Manage referrals and contribute to integrated care by working collaboratively with multiple disciplines and teams, ensuring clinical excellence through research, teaching, consulting, and leadership within the nursing profession.
  • Work autonomously and in partnership with healthcare professionals in an advanced nursing role, meeting the clinical demand within their defined scope of practice and formulary, including patients with complex health conditions.
  • Develop expertise in managing chronic and complex health conditions and support best practice emergency/inpatient care plans and discharge coordination for Aboriginal and Torres Strait Islander patients with chronic or complex medical diagnoses presenting to Coffs Harbour Health Campus and Galambila Aboriginal Medical Services.
  • Cultivate culturally safe partnerships with priority population groups within the Coffs Clinical Network community, including Aboriginal and Torres Strait Islander people, individuals living with disabilities, people experiencing homelessness or financial difficulties, older adults, and culturally diverse groups.
  • Coordinate health activities and refer patients to relevant internal and external community agencies and services based on their specific needs.
