Job Title: Care Manager

This role is pivotal in facilitating person-centred care through commitment to case management, client support and astuteness in assessment, care planning and review.

The position works within In Home Support programs providing care and services to elders living in the community.

Key Responsibilities:
  • Conduct initial assessments, develop care plans, and monitor elders according to MiCare Home Care Policies and Commonwealth Government Aged Care guidelines and Charter of Aged Care Rights.
  • Liaise with internal and external stakeholders to ensure elders receive appropriate services based on their needs and program guidelines.
  • Organize case and planning meetings as required, ensure accurate documentation and follow-through on elder health, care and service delivery.
  • Develop individualized budgets for elders with a Home Care Package, ensuring spending compliance within Home Care Package Guidelines.
  • Initiate referrals and permanent placements where necessary.
  • Provide support and advocacy for elders and carers as needed.
  • Maintain quality review processes to ensure documentation and processes meet compliance requirements.
  • Develop working relationships with relevant agencies and networks to promote the organization.
  • Respond to phone and online inquiries, provide public education to potential clients.
About You:
  • Demonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues.
  • Clear understanding of Aged Care Quality Standards.
  • Solid understanding of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program and Veterans Home Care).
  • Proactive, reliable, with excellent communication skills and attention to detail.
  • Ability to work collaboratively and be a supportive team member.
  • Ability to meet compliance requirements, including current First Aid and CPR certificate, valid Victorian drivers licence with insured and registered vehicle, ability to provide evidence of current flu and COVID-19 vaccination.
Qualifications:
  • Tertiary qualification in Allied Health, Nursing or Social Work.
